Miocardiopatía de Takotsubo: una imitadora del síndrome coronario agudo
Resumen
La miocardiopatía de Takotsubo es una entidad clínica caracterizada por disfunción sistólica transitoria del ventrículo izquierdo que simula un síndrome coronario agudo, predominando en mujeres posmenopáusicas y asociándose frecuentemente a estrés físico o emocional. Su diagnóstico continúa siendo un desafío debido a la similitud de su presentación clínica con los síndromes coronarios agudos, por lo que requiere una adecuada integración de hallazgos clínicos, electrocardiográficos, bioquímicos y de imagen.
Se presenta el caso de una mujer de 76 años, con antecedentes de hipertensión arterial, diabetes mellitus tipo 2 e hipotiroidismo, quien consultó por dolor torácico opresivo y disnea. El electrocardiograma evidenció inversión simétrica de ondas T en derivaciones precordiales, mientras que los biomarcadores cardíacos mostraron elevación significativa de troponinas y NT-proBNP, orientando inicialmente hacia el diagnóstico de síndrome coronario agudo sin elevación del segmento ST.
El ecocardiograma transtorácico reveló fracción de eyección preservada con acinesia anteroapical y alteración moderada del strain longitudinal global, hallazgos compatibles con síndrome de Takotsubo. El puntaje diagnóstico InterTAK fue altamente sugestivo. La cinecoronariografía descartó enfermedad coronaria obstructiva significativa y la ventriculografía demostró el patrón clásico de balonamiento apical, confirmando el diagnóstico. Debido a la presentación clínica típica y a la favorable evolución, no se consideró necesaria la realización de resonancia magnética cardíaca. Tras excluir una etiología isquémica obstructiva, se suspendió la doble antiagregación plaquetaria y se optimizó el tratamiento médico con betabloqueantes y medidas de soporte. La evolución clínica fue favorable, con recuperación completa de la contractilidad segmentaria y normalización de la función ventricular en el seguimiento ecocardiográfico realizado al mes del evento.
Este caso destaca la importancia del diagnóstico diferencial oportuno de la miocardiopatía de Takotsubo en pacientes con sospecha de síndrome coronario agudo, así como el valor de la evaluación multimodal para establecer el diagnóstico, orientar el tratamiento y optimizar el seguimiento clínico.
